1. Agreement to Terms

These Terms of Service (“Terms”) govern your access to and use of the website located at https://www.www.alcok.com, including all related pages, forms, online scheduling tools, content, and services (collectively, the “Site”) operated by Advanced Laser & Cataract Center of Oklahoma (“ALCOK,” “we,” “us,” or “our”).

By accessing or using the Site, booking an appointment through it, submitting a form, or opting in to receive communications from us, you agree to be bound by these Terms and our Privacy Policy. If you do not agree, please do not use the Site.

2. About ALCOK

ALCOK is an ophthalmology practice based in Oklahoma City, Oklahoma, specializing in cataract surgery, refractive lens exchange (lens replacement), the EVO ICL procedure, glaucoma treatment, eyelid surgery, corneal transplant, pterygium treatment, and related surgical and medical eye care services.

3. Eligibility

You must be at least 18 years of age, or accessing the Site under the supervision of a parent or legal guardian, to use the Site or submit information through it. By using the Site, you represent that you meet this requirement.

4. Medical Disclaimer — Important

The content on the Site is for general informational and educational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. Reading information on the Site, contacting us through a form, or scheduling an appointment does not create a doctor-patient relationship between you and ALCOK or any of our providers. A doctor-patient relationship is established only after a formal evaluation in our office.

  • Always seek the advice of a qualified physician for any questions about a medical condition.
  • Never disregard professional medical advice or delay seeking it because of something you read on the Site.
  • If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use the Site, chat, SMS, or email to communicate emergencies.
  • Outcomes, recovery times, and suitability for procedures vary by individual. Testimonials, statistics, and “before and after” content on the Site reflect specific patients’ experiences and are not a promise or guarantee of your results.

5. Appointments and Online Scheduling

The Site offers online appointment booking and self-evaluation tools (provided by third-party platforms such as Klara and Typeform). Submitting an appointment request is not a confirmed appointment until our office contacts you to confirm. Appointment availability, cancellation rules, and no-show policies will be communicated by our staff and may be governed by separate office policies.

6. Communications and Consent

6.1 General Consent

By providing your contact information through the Site, you consent to be contacted by ALCOK by phone, email, and/or text message at the contact information you provide, for the purposes described in our Privacy Policy.

6.2 SMS / Text Messaging

6.2.a Consent. By providing a mobile number on any ALCOK form, online booking tool, web chat, or by texting us first at 405-835-3418, you expressly consent to receive recurring autodialed and prerecorded text messages from ALCOK at that number, including appointment communications and, where you have separately opted in, marketing messages. Consent is not a condition of purchasing any service or receiving care. See Section 4 of our Privacy Policy for full SMS terms.

6.2.b Cancellation. You can cancel the SMS service at any time. Just text “STOP” to 405-835-3418. After you send the SMS message “STOP” to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and we will start sending SMS messages to you again.

6.2.c Help. If you are experiencing issues with the messaging program, you can reply with the keyword “HELP” for more assistance, or you can get help directly at contact@advancedokc.com.

6.2.d Message Frequency and Rates. Message frequency varies based on your interaction with ALCOK, such as appointment scheduling, confirmations, reminders, and follow-up communications. You may receive up to 10 messages per month. Message and data rates may apply for any messages sent to you from us and to us from you. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.

6.2.e Carrier Liability. Wireless carriers (including but not limited to AT&T, Verizon, T-Mobile, US Cellular, and other participating carriers) are not liable for delayed or undelivered messages. ALCOK makes no guarantee regarding the delivery, timing, or receipt of any text message and is not responsible for any failure, delay, or error in transmission caused by your carrier, device, network conditions, or any other circumstances outside our reasonable control. You are solely responsible for any message and data charges based on your mobile plan. Do not rely on SMS for time-sensitive or emergency communications — call 911 or our office directly.

6.3 Email

You may receive transactional and, where opted in, marketing email from ALCOK. Every marketing email includes an unsubscribe link.

7. Insurance, Fees, and Payment

Information on the Site about insurance acceptance, financing, costs, and procedures is general in nature and subject to change. Specific coverage, eligibility, and pricing must be verified directly with our office and your insurance carrier. We are not responsible for misunderstandings arising from general informational content.

8. Intellectual Property

All content on the Site — including text, graphics, logos, images, video, copy, layout, the ALCOK name, and trade dress — is the property of ALCOK or its licensors and is protected by United States and international copyright, trademark, and other intellectual property laws.

You may view, download, and print Site content for your personal, non-commercial reference only. You may not:

  • Reproduce, republish, distribute, or sell any Site content
  • Use ALCOK’s name, logos, or branding without prior written permission
  • Modify or create derivative works from Site content
  • Use any data mining, scraping, or automated extraction tools on the Site

9. User Conduct

When using the Site, you agree not to:

  • Submit false, inaccurate, or misleading information
  • Use the Site to harass, threaten, defame, or harm any person
  • Attempt to interfere with, disable, or circumvent the security of the Site
  • Upload viruses, malware, or other harmful code
  • Access or attempt to access areas of the Site not intended for public use
  • Use the Site for any illegal purpose or in violation of applicable laws

We reserve the right to refuse service, block access, or remove content at our discretion.

10. Submitted Information

You are responsible for the accuracy of any information you submit through the Site. Please limit the personal health details you submit through web forms, chat, SMS, or email — these channels are not secured for the transmission of Protected Health Information. For sensitive medical communications, call our office directly.

By submitting information, you grant ALCOK a non-exclusive, royalty-free right to use it for the purposes described in our Privacy Policy.

11. Third-Party Services and Links

The Site may contain links to or integrations with third-party services, including but not limited to Klara, Typeform, GoHighLevel, Google, Meta (Facebook, Instagram), and YouTube. These services have their own terms and privacy policies. ALCOK is not responsible for the content, practices, or policies of any third-party site or service.

12. Disclaimer of Warranties

THE SITE AND ALL CONTENT, INFORMATION, AND SERVICES PROVIDED THROUGH IT ARE PROVIDED ON AN “AS IS” AND “AS AVAILABLE” BASIS. TO THE FULLEST EXTENT PERMITTED BY LAW, ALCOK DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WITHOUT LIMITATION WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, ACCURACY, AND UNINTERRUPTED OR ERROR-FREE OPERATION.

WE DO NOT WARRANT THAT THE SITE WILL BE SECURE, FREE OF VIRUSES, OR AVAILABLE AT ALL TIMES.

13. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, ALCOK, ITS OWNERS, PHYSICIANS, EMPLOYEES, AGENTS, AND AFFILIATES SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, PUNITIVE, OR EXEMPLARY DAMAGES, OR ANY LOSS OF PROFITS, DATA, OR GOODWILL, ARISING OUT OF OR RELATING TO YOUR USE OF, OR INABILITY TO USE, THE SITE — EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.

This limitation does not apply to professional medical care, which is governed by applicable Oklahoma law and our separate patient agreements.

14. Indemnification

You agree to indemnify, defend, and hold harmless ALCOK and its physicians, employees, agents, and affiliates from any claims, damages, losses, liabilities, and expenses (including reasonable attorneys’ fees) arising out of or related to your misuse of the Site, your violation of these Terms, or your violation of any rights of a third party.

15. Governing Law and Jurisdiction

These Terms are governed by the laws of the State of Oklahoma, without regard to its conflict of laws principles. You agree that any dispute arising out of or relating to these Terms or your use of the Site shall be brought exclusively in the state or federal courts located in Oklahoma County, Oklahoma, and you consent to the personal jurisdiction of those courts.

16. Changes to These Terms

We may update these Terms from time to time. Material changes will be posted on this page with an updated “Last Updated” date. Continued use of the Site after changes are posted constitutes acceptance of the revised Terms.

17. Severability and Entire Agreement

If any provision of these Terms is found unenforceable, the remaining provisions remain in full effect. These Terms, together with our Privacy Policy, constitute the entire agreement between you and ALCOK regarding your use of the Site.
